NOTICE OF DATA SECURITY INCIDENT

Eye Care Center of Snohomish is providing notice of a data security incident that involved certain patient information. We want to provide information about the incident, the steps we have taken in response, and steps individuals may take to help protect their information.

What Happened? On August 12, 2026, Eye Care Center of Snohomish detected unauthorized activity involving portions of our network. We promptly engaged outside cybersecurity specialists, secured the environment, and began an investigation. Based on the forensic evidence, we believe an unauthorized actor gained access to a limited number of files in our system on or about July 28, 2026. Eye Care Center of Snohomish then conducted a thorough review to identify the potentially affected files and the information and individuals involved. On or about September 28, 2026, we confirmed the impacted population. At this time, there is no indication that the affected information has been misused.

What Information Was Involved? The information involved varied by individual and may have included name, address, date of birth, and medical information, including diagnosis, lab results, medications, and/or other treatment information. The information involved was not the same for every person.

What We Are Doing. Eye Care Center of Snohomish began investigating the incident as soon as we learned of it. We engaged with the relevant law enforcement agencies and initiated our incident response process to contain the activity, restore affected systems, and review and strengthen our safeguards.

What You Can Do. We encourage individuals to remain vigilant against incidents of identity theft and fraud by reviewing account statements and monitoring free credit reports for suspicious activity.
