Helpful guidelines for appointments, prescriptions, records, eyewear adjustments, and patient support.
Our goal is to provide a calm, professional, and welcoming experience for every patient. These guidelines help our team keep appointments organized, protect patient information, and provide the best care possible.
Appointments
Please arrive on time for your appointment so we can complete check-in, testing, and your exam as smoothly as possible. If you are running late, please contact our office so we can let you know the best next step.
Referrals and Prior Authorizations
Some insurance plans require a referral or prior authorization before your visit. If your plan requires one, it is the patient’s responsibility to help request this from their primary care provider or insurance plan before the appointment.
Eyewear Adjustments
Our optical team is happy to help with eyewear adjustments. If your glasses are slipping, crooked, uncomfortable, or need a better fit, we can usually help adjust them. Some repairs may depend on frame condition and parts availability.
Cancellations and Rescheduling
If you need to cancel or reschedule, please contact us as soon as possible. This allows us to offer the appointment time to another patient who may be waiting for care.
Prescriptions
Glasses and contact lens prescriptions may be requested through our Patient Resource Center. Contact lens prescriptions may require that contact lens fit and follow-up requirements are complete before release.

Insurance Information
Please provide your most current insurance information before your appointment. Insurance benefits and requirements can change, so it is important that we have accurate information on file.
Medical Records
Medical records requests must be reviewed and processed according to privacy requirements. Please allow time for our office to complete your request.
Billing Questions
Billing questions are reviewed through our billing question form in the order they are received. Please submit billing questions online so our team can review your account and respond accurately.
