Important Insurance Notes

Apple Health / Coordinated
Care / Molina
At this time, we do not accept Apple Health plans, including Coordinated Care, Molina, or other Apple Health plans
Kaiser Plans
We do not accept Kaiser Core. We may accept Kaiser PPO plans.
Please contact your plan if you are unsure.
UnitedHealthcare / Spectera Vision
We accept UnitedHealthcare for eye exams only. We do not accept Spectera for glasses or contacts.
Insurance Need a Referral.
Humana insurance plans require a valid referral from your primary doctor before your appointment.
Vision Insurance vs Medical Insurance
Many visits at our office involve medical eye care, not just a routine vision check. Our doctors evaluate the health of your eyes and may monitor or diagnose conditions such as cataracts, glaucoma concerns, diabetes-related eye concerns, dry eye, flashes or floaters, eye pain, infections, autoimmune-related eye issues, and other medical findings.
Vision Insurance
Usually helps with routine vision benefits
May cover routine vision exams
May provide benefits toward glasses or contact lenses
Usually does not cover medical eye conditions, diagnostic testing, monitoring, or treatment
Medical Insurance
Used for eye health concerns, symptoms, diagnoses, testing, monitoring, and treatment
May apply when the doctor finds or monitors a medical eye condition
Includes conditions such as cataracts, glaucoma concerns, diabetes-related eye concerns, dry eye, eye pain, flashes or floaters, infections, autoimmune-related eye issues, and other medical findings
Many visits in our office are billed through medical insurance because our doctors provide comprehensive medical eye care

Referrals & Prior Authorizations
Some insurance plans require a referral or prior authorization before your appointment. Referrals are usually requested through your primary care provider or insurance plan.
