How to Request a Referral
Contact your primary care provider and ask them to send a referral to Eyecare Center of Snohomish before your appointment.
Send Your Referral
Have your provider send the referral to our office using the contact information included in the message below.

Please send a referral to Eyecare Center of Snohomish for evaluation and treatment of my eye care needs.
If allowed by my insurance plan, please include authorization for any medically necessary office visits, diagnostic testing, imaging, procedures, and follow-up care recommended by the eye doctor.
Please do not limit the referral to only one visit or one basic exam if broader authorization is available.
Please include:
- Approved number of visits
- Diagnosis code, if required
- Authorization or referral number, if applicable
- Effective date range of the referral
- Any limitations or restrictions
